Phones:

  • Telephones are answered by our staff during office hours.
  • Emergencies: For life threatening emergencies, call 911. If you have an urgent problem after hours, please go the nearest urgent care/ER, don’t call our office. During normal working hours, calls will be returned by the end of the working day. No over the phone treatments are provided, appointments are required.

Office Hours

  • Test Results: For test results call 281-554-2846 and speak with a nurse. An appointment is usually needed to discuss any results.
  • Prescriptions: Refill requests are honored for active patients only. All prescription refills should be requested during normal office hours. Please contact pharmacy for refill requests to be faxed at 281-724-1321 and allow 48 hours for a response from our office. Some refill requests will require an appointment.

Appointment:

  • For appointments, please call 281-554-2846. As a courtesy to other patients and staff, please notify the office if you are running late for the appointment.
  • There will be charge for NO-SHOW appointments of $25.00, if you don’t call and cancel 24 hours in advance.
  • Unaccompanied minors will not be seen unless authorized by parents or legal guardian by an authorized note.

Insurance

  • Prior to your appointment, please check your insurance information so you will be informed about referrals, co-payments, and any deductibles required at the time of the visit. We also accept Visa, Master, discover and American Express cards.
  • For your first visit, please bring your insurance card and arrive 20-30 minutes early to complete the necessary patient information forms, or bring filled forms with you.                                 
  • Referrals: please allow 48 hours for referral process. If you are requesting to be referred, please bring the required information with you at the time of appointment.
  • Your health insurance contract is between you and your insurance company. Any complaints regarding your coverage should be directed to your carrier.

What do we need from you?

  • To inform the Office of any changes in insurance, employment, demographic information or information with other care/service givers, failure to do will result in denial of the claim.
  • To arrive on time for scheduled appointments and cancel, when necessary, with a phone call.
  • To follow the recommended treatment plan and inform office of any physical or mental impairment requiring special accommodation.
  • Provide with any previous medical records.
  • If your insurance company requires a referral to a specialist or authorization for a diagnostic test, you are responsible for obtaining that referral before seeing a specialist or having a diagnostic test performed. In most cases, your primary care physician must see and treat a condition before a referral can be given.
  • There is a fee for filling forms or paperwork completed by physicians. The fee varies according to the forms/documents needed. Forms and paperwork can take up to 7 business days, so please allow sufficient time.
  • You will be automatically considered inactive patient after 3 missed appointments without any advance notices or passage of 6 months without any appointment, with no response to office attempts for scheduling appointments.
  • Any abusive language/behavior may result in discontinuing of Physician-patient relationship.

What should you expect from us?​

  • Your clinician is responsible for providing you with quality professional service, which includes treating you with respect, maintaining your confidentiality, and informing you about your condition/diagnosis and treatment options.
  • Your clinician might consult other Physicians to provide better care, which will be discussed with you.
  • Your Physician takes the responsibility for your medical records. Written permission is required to release any information to another agency. The only exception to this policy occurs when the clinician has concerns about possible abuse/neglect or when the clinician believes there is a serious threat of self-harm or harm to others, as required by law.
  • To discuss laboratory/imaging results at follow-up appointments and offer a treatment plan
  • Your satisfaction is very important to us, if your dissatisfied with this office, we ask that you speak with your clinician about your concerns.
